Applications and Resources
We are happy to share valuable information with you to help care for your pet. Below you'll find links to information on surgical recovery, vaccinations, declawing and more.
Applications
Thank-you for your interest in being welcomed as an EVAH Volunteer and/or an EVAH Rescue Client! At this time only our new HAMILTON location is accepting applications from non profits/shelters/rescues. Download the applicable application from the links below, or e-mail evahofficehamilton@gmail.com for further information.
All public low income clients may contact the clinic closest to their location and enquire as to whether or not we are able to accept new patients in any of our public programs.
Due to COVID, we are not in the position to consider new volunteers in any of our clinic locations currently. Should this change in the near future, we will post an update on social media and our website.

VCPR FOR NEW CLIENT(PATIENT)
EVAH’s “Scope of Services” include general medical and surgical services for dogs and cats, on an appointment basis, between the hours of 830am for surgery intake or beginning at 10am for medical /
technical appointments, running to 5pm, Monday or Tuesday through Friday. Veterinary medicine will be delivered according to the current standards of the profession: veterinarians are accountable to
provincial governmental regulations, as well as our governing body, the College of Veterinarians of Ontario.
All clients must be either low-income status, according to the guidelines listed on our website (evah.ca), animal welfare / rescue organizations (separate application process) or caregivers of outdoor
cats. Public clients will be re-screened for income status with each new pet in the home.
A veterinarian and client agree to establish a VCPR, based on these “Scope of Services”, by agreeing to this appointment with an EVAH veterinarian. Please advise the name and address of any veterinary hospital(s) that has provided care for your pet in
the past, such that prior medical records can be requested. If records are not received, this may result in ‘uncoordinated services” and may put your animal at risk.
You are welcome to continue to request appointments through the lifetime of your pet, in times of good health and sickness, assuming that all communications continue to be respectful to all of our team
members.
The VCPR is discontinued when the client moves away and wishes veterinary care more locally, or sadly, when the pet passes away or is euthanized. On very rare occasions, we unfortunately need to consider to discontinue the VCPR, for the health and
safety of our team members. If a staff or volunteer feels a client is either verbally or potentially physically abusive or threatening, or demands unnecessary medications or services your VCPR will be terminated.
